Welcome to Dr. David Sugar’s Office! We are honored you have chosen us and we are here to serve you. To make an appointment, please call our office at 941.556.6900. Prior to your scheduled appointment, we can email you New Patient paperwork. You can either print up the paperwork to complete at home or complete in the office.
Appointments…
Call our office during business hours to schedule appointments, change appointments, and schedule surgeries. Even if you have reached our voicemail during business hours, leave a message and we will call you back as soon as possible.
EFFECTIVE February 1, 2025—we will no longer be accepting any insurance.
Monday-Thursday, 8:00 am until 4:00 pm.
We are closed for lunch Monday through Thursday, 11:30 am until 1:00 pm.
Please know that although our phones are off, we monitor all messages left during lunch. We are charging a $50.00 fee for those who NO SHOW for their appointment. We simply are asking that you give us the courtesy that we provide each of you by simply calling us to let us know you to need to cancel or reschedule.
Cancellations…
If you need to cancel an appointment, we require a courtesy call prior to the appointment.

What to bring…
- New Patient Forms
- Medical Records, Medication List, Allergies, X-Rays, MRI’s, etc.
- Current Insurance Card (in case we need to refer you out) & Driver’s License
- List of any questions you may have for the provider
- All payments are due at the time of service.
Insurance and Payment…
