To expedite your first visit, we encourage you to sign into our new patient portal and fill out the forms prior to your first appointment. In order to do this, please contact our office, provide us an email address and we will assign you an account number for security purposes and email you the link. Welcome to Sugar Orthopaedics! We are honored you have chosen us and we are here to serve you. To make an appointment, please call our office at 941.556.6900. Prior to your scheduled appointment, we can email you New Patient paperwork. You will need your account number to complete this process and it will be given to you at the time of scheduling. Appointments…

Call our office during business hours to schedule appointments, change appointments and schedule surgeries.  Even if you have reached our voicemail during business hours, leave a message and we will call you back as soon as possible.

Monday-Friday, 8:00 am until 4:00 pm. We are closed for lunch, 11:30 am until 1:00 pm. We are now charging a $25.00 fee for those who NO SHOW for their appointment.

Cancellations…

If you need to cancel an appointment, we require a courtesy call prior to the appointment.

What to bring…

  • New Patient Forms
  • Medical Records, Medication List, Allergies, X-Rays, MRI’s, etc.
  • Current Insurance Card & Driver’s License
  • List of any questions you may have for the provider
  • Specialist Co-Pays are expected at the time of your visit along with your Deductibles.

Insurance and Payment…

Sugar Orthopaedics works with a wide variety of insurance providers. Specialist co-pays are due at the time of your appointment. After your insurance processes your claims, payments are due. Please call us for more information. *If your insurance requires a referral to see a specialist, the referral must be faxed directly to our office from your assigned Primary Care Physician BEFORE we can schedule any appointment. This is specific but not limited to Freedom & Optimum Plans, Medicare Advantage Plans and all Medicaid and most HMO plans.

  • We do not accept Aetna Coventry HMO, Aetna MCR HMO Plans, Aetna through the Exchange, AmBetter Health, AmeriHealth, BCBS HMO, BCBS Blue Select, Molina Medicaid, PHCS/Multiplan, Prestige, Staywell Medicaid, UHC Medicare Advantage or Humana plans at this time.
  • Regarding Medicaid…we see limited Medicaid plans for children who live in Sarasota from our Referring Dr’s. Referrals are required prior to scheduling.
